Should a couple tell each other about whether they had past sexual relationships, and if so, should they reveal the previous partners identity if it is an individual with whom they are still in contact?

These kinds of revelations should be made with extreme discretion and without much detail. If there is no threat or lingering ongoing inappropriate relations with the person with whom sexual relations had occurred, there is no reason to reveal this person's identity. It is enough to just say, "I had made some bad decisions in my past that I regret, but have since disclosed my shortcomings to my spiritual father, repented, and was absolved by the grace in God." No one should press the other for more information or details.
